Affordable Care Act (ACA) Form 1095-C - Notice of Availability

The Paperwork Burden Reduction Act and the Employer Reporting Improvement Act enacted on January 1, 2025 introduced several changes to simplify the ACA reporting process for employers, including the requirement to provide IRS Form 1095-C (Employer Provided Health Insurance Offer and Coverage) only upon request. This means you will not automatically receive this form each year. Instead, you can request it, if needed.

What You Need to Do:

If you do not need a copy of your Form 1095-C, you do not need to do anything. This will apply to most individuals.
If you need a copy of your Form 1095-C, you may request it by contacting the individual below. We will provide the form within 30 days of your request.
